Tirzepatide and Body Composition: Was the Lost Weight All Fat?
A scan substudy separates body weight, fat mass and lean mass.
Why is the bathroom scale not enough?
A scale adds everything together. It cannot tell whether a change came from fat, lean tissue or fluid. A body-composition measurement separates compartments. The SURMOUNT-1 substudy used DXA, a scan-based method, to compare participants at the start and at week 72. [1]

What decreased?
In the pooled tirzepatide group, average changes were −21.3% in weight, −33.9% in fat mass and −10.9% in lean mass. Placebo values were −5.3%, −8.2% and −2.6%. The authors estimated that roughly three quarters of weight lost was fat and one quarter was lean mass. [1]
Why are 33.9% and 75% different?
They divide by different starting amounts. A fat-mass reduction asks how much fat disappeared relative to starting fat. The 75% split asks what share of the lost weight was fat. In an illustrative 10 kg loss split 75/25, 7.5 kg would be fat and 2.5 kg lean mass. This is a calculation example, not an individual result.
Does lean-mass loss equal strength loss?
