Understand the prediabetes subgroup, diabetes incidence and absolute differences in the long-term SURMOUNT-1 analysis.

Why this analysis concerns 1032 people
SURMOUNT-1 enrolled 2539 participants with obesity, including 1032 who also had prediabetes. This long-term analysis concerned the latter subgroup. Its risk figures should not be applied to all healthy people, all people with diabetes or the entire initial trial population.
Diabetes incidence at 176 weeks
The paper reported a type 2 diabetes diagnosis in 1.3% of pooled tirzepatide groups versus 13.3% with placebo. The figure uses a zero-based vertical axis. These are observed proportions for the study population, not individual predictions or a complete description of the follow-up trajectory.

| Condition | Incidence (%) |
|---|---|
| Tirzepatide | 1.3% |
| Placebo | 13.3% |
Absolute differences and modeled risks differ
Subtracting 1.3% from 13.3% gives an absolute difference of 12.0 percentage points. A relative comparison calculated from these proportions is not the same statistic as a modeled hazard ratio. Do not merge a hazard ratio, diabetes incidence and weight-change percentage into one undefined effectiveness rate.
Weight and later follow-up use different endpoints
At week 176, mean weight changes were −12.3%, −18.7% and −19.7% across the tirzepatide dose groups, versus −1.3% with placebo. After a further 17 weeks off treatment, diabetes proportions were 2.4% and 13.7%. Keep endpoints and timepoints matched when comparing results.
Include the adverse-event context
The paper reports gastrointestinal events as common adverse events, mostly mild to moderate and occurring mainly during dose escalation in the first 20 weeks. Individual applicability requires a complete benefit–risk assessment. This explanation does not provide a personal medication plan.
Clinical findings do not validate a catalog batch
The Tirzepatide page supports research-material and quotation enquiries. Clinical-trial formulations and catalog materials require separate identification. A shared name alone does not make trial outcomes evidence of a particular catalog batch's efficacy or safety.
